Transcranial Doppler (TCD) ultrasound has emerged as a valuable non-invasive tool to detect PFO and is listed in the guidelines.
TCD is a non-invasive ultrasound technique for real-time, dynamic monitoring of cerebral blood flow in major intracranial arteries with high temporal resolution. It uses low-frequency (≤2 MHz) ultrasound waves to penetrate the skull and detect the Doppler shift of moving red blood cells in cerebral vessels. The key parameters measured include mean flow velocity, pulsatility index, and detection of microembolic signals (MES). TCD has higher sensitivity than Transthoracic echo (TTE) for embolus detection and can provide a safe, less invasive alternative for detection of right-to-left shunt.
